Headache is the most common presenting symptom of cerebral venous thrombosis (CVT) present in 70-90% patients. It may be the sole symptom in some of patients. A 28-year-old male patients presenting with sudden onset severe headache was diagnosed with CVT. His initial laboratory study was unremarkable, he began taking oral anticoagulation therapy as diagnosed with idiopathic CVT. After he stopped taking anticoagulation, CVT recurred. We investigated the causative factors such as hypercoagulable state, he diagnosed with antiphospholipid syndrome. Because headache may be the only neurologic symptom of CVT, it is needed to differential diagnosis from other cause of headache and it is important to investigate the underlying prothrombotic conditions even the initial test was negative in case of recurrent CVT.
